Back tag number question

I am sending in  for a permit for the lottery to hunt for pheasant at one of the local state parks on day when permit is needed. It says to include back tag numbers, which has top and lower numbers in large bold print, and middle row has 4 numbers that are small in print. Should I include the small print numbers as part of my back tag numbers? I want to be sure before I fill out index card and mail the info in by deadline date. Just seemed odd having those numbers in such small print.
