That's not "Hollywood", it's anti-smoking groups that have pressured the MPPA, broadcasters and advertisers to include the warning since 2007. Hollywood collectively (if you can say such a thing) doesn't care. They're in the business to make money and to occasionally create a piece of filmic art.
